ACT offers its services to both the adult and pediatric population. For an adult client, home health care services is initiated either by the client or the family when the client can not take care of their medical needs due to a chronic condition or a change in their health due to an illness, accident etc. Often a physician, nurse, hospital discharge planner or case manager suggests obtaining professional help at home to assist with the client’s health care needs.

For our pediatric clients—our highly skilled and caring staff can take care of children of different ages, including children who require very complicated care in various settings including home or school.


Private Duty Nursing Services


Private Duty Nursing Services are skilled nursing services that are provided on a continuous basis to clients with complex medical needs of any age in their home. PDN services are provided under the direction of a written individualized order/ plan of care signed by the recipient’s attending physician. These services are provided by Registered nurses (RNs) and / or licensed practical nurses (LPNs). These are services that cannot be performed safely and effectively by nonprofessional personnel in the client’s home. The intricacy of a patient’s medical condition and required course of treatment determine whether care should be provided by an RN or can be provided by an LPN.

Home Health Aide Services


HHA Services is home care services provided by Certified Nursing Assistants (CNAs) and home health aides. The services provided vary according to our client’s needs and the level of support offered by the family. Home Health aides provide non-medical health care at a client’s home. Home health aides assist clients with Activities of Daily Living (ADLs) such as getting in and out of bed, walking, bathing, toileting, and dressing, meal preparation. Some aides have received special training and are qualified to provide more complex services under the supervision of a nursing professional.




Skilled Nursing Visits


A skilled nursing visit is a nurse visit by an RN or LPN to initiate and complete a professional nursing task as assessed to meet the person’s needs in his or her home. A skilled nursing visit can include the following services:

  • Hands on nursing care
  • Health care training and education of the patient and/or their family with regards to treatment and care of a disease
  • Observation and assessment of our client’s physical status
  • Wound care / monitoring
  • Catheter care
  • Injection teaching
  • Nutritional assessment and weight check
  • Enteral feeding technique
  • Medication compliance
  • Post-operation incision check

Well Mom Well Baby (WMWB)


A WMWB visit is a nurse visit to the client’s home within 48 hour of discharge from the hospital after delivery to make sure mom and the baby are adjusting to being home. The RN will do an assessment for mom as well as the baby. The RN will also educate Mom about other resources that are available to assist and support her during this transitional time.
